$10 donation - Terms and conditions

1. In these terms and conditions “Audika” means Audika Australia Pty Ltd (ABN 40 061 969 006).
2. Offer only available to Country Women’s Association of WA (CWA WA) members, CWA WA members’ spouse/domestic partner and CWA WA members’ parents, who are 26 years of age or older.  Audika clients who have been fitted with hearing aids are not eligible for the offer. An Audika client is a person who has received services from a from a clinic trading under the Audika brand or by an Audika clinician (including services provided by Sonic Innovations Pty Ltd trading as Audika Australia). “Services” means professional services of any kind and includes previous hearing tests/checks/screens and any other completed appointment type.
3. Audika will donate $10 to the CWA WA for every client who meets the eligibility criteria in clause 2, books and attends a hearing test or hearing screen with Audika before the expiry/termination of the offer and who advises they are a CWA WA member or spouse/domestic partner/parent when booking the test/screen (“CWA WA Donation Offer”).
4. Employees of Audika and any of its related companies are not eligible for the CWA WA Donation Offer. 
5. Limit of one (1) $10 donation to CWA WA per person per year. Donations will be paid by Audika to CWA WA.
6. The CWA WA Donation Offer may be terminated by Audika at any time.  In the event of termination, Audika will pay the $10 donation for all hearing tests/screen attended up to the date of termination.
7. CWA WA Donation Offer is only available for tests and screens conducted in an Audika clinic (including visiting sites) and is not available for the on-line, self-hearing test at the audika.com.au website or any tests/screens conducted outside the clinic environment (for example, at community events).
8. Clients must mention that they are a CWA WA member or spouse/domestic partner/parent of a CWA WA member, and all contact details requested must be provided when booking to be eligible for this offer.  Audika reserves the right to verify CWA WA membership details.
9. To receive the CWA WA Donation Offer clients must attend their booked appointment. $10 donation will not be made if appointment is cancelled or a no-show.
10. For the purposes of administering the CWA WA Donation Offer, it is necessary for Audika to collect personal information about you and to disclose your personal information to CWA WA.  For further information regarding how we manage personal information, refer to our privacy policy at www.audika.com.au/privacy-policy.
